Complete form to right.  The information goes to Membership when you click the SEND button

• Place Amount you are donating in the Paypal Donation Box.  This can be for membership, purchase of items or a donation, period.  Whatever amount you wish to send to MHA.
• If membership donation, indicate if you are new or renewing, your membership level (Family $40, Single $30 and Business $50), and your contact information.
• If a business membership, also fill in Business name and give us information about your business (website, history etc).  This is to assist in completing your webpage on the MHA website.
• Please indicate if you wish to volunteer for the board or help in various ways
• Also indicate if you use the Preserve
• Comments are always appreciated.
• When you click the Send button the information on the form will be sent to the membership chair (Phyllis).

PAY-when you click on the Donate button, you start the procedure to send money to MHA via Paypal.

• Then click the Pay Pal donate button and follow instructions and make your payment. The treasurer (Barbara) gets notified that the your donation was received by MHA and deposited to MHA's checking account.
